This plea serves to assist in maintaining and improving the quality of healthcare services in the institution through advancing sustained relations with its customers within the catchment, ascertaining that the hospital meets the set turnaround time and for effective and accurate data capturing.
Therefore, the hospital management requests that the following documents should be produced when coming for a consultation:
• Identity document
• Child’s birth certificate
• Salary advice
• UIF card /proof of unemployment
• Details of next of kin
• Social pension card
• Medical aid card
• Proof of address
• Employer’s report if injured on duty
